Covid-19 Pfizer Vaccine Update
We will be receiving a shipment of Covid-19 Pfizer vaccine on Wednesday, May 26th. This vaccine can be administered to anyone 12 years and older. We will be administering the vaccine on specific days, please call our office to schedule an appointment.
After you schedule your appointment with our office, you will need to complete a Covid 19 Vaccine Form online. Please click the attached link to complete the form – https://forms.ny.gov/s3/vaccine. This form will need to be shown at your appointment time. You may either save it to your cell phone or print the form.
After your first Covid-19 vaccine, you will be given an appointment to return to our office for your second vaccine.
